This system provides features to view all information and records, sharing information, make the process of editing, updating and retrieving data become easier for HSE administrator. In developing this web based system, the waterfall model is used. This system is developed using Macromedia Dream Weaver, PHP, Apache and MySQL. The finding shows that the prototype has become a communication medium for all parties involved.
