Data visualization in graphical form can help organization to view the summary of their data, which facilitates better pattern recognition to derive any decision-making solution. Its important aspect to include in a survey or questionnaire application as the result of the data collection should be presentable in different format. E-Latihan System was developed as a digitalized version of traditional pen and paper evaluation form management, but the current data reporting modules lack in providing the administrator with better analysis representation. The objective of this paper is to outline general requirements for a data visualization reporting module based on E-Latihan's case study in order to address the stated problem. At the same time, to examines the use and impact of data visualization to Perak Syariah Judicial Department. The result form this study shows that, data visualization able to help identified personnel in making quality decision making and time fact based decision. © 2018 IEEE.
